Request check card for existing account

Use this request form if you:

Have an existing Bank of America checking account but don't have a check card
Need a replacement check card
Want to change your check card type
Want a check card for an account owner who does not currently have one (Note: only one check card per account owner may be issued)

If approved, your new check card will be mailed to the primary account address within 10 days.

If you don't have one yet, you can open a checking account now and receive a check card with your new account.
